The Engineer Who Forgot: What Really Protects the Brain from Dementia

“Doctor? Can you tell me what protects me from dementia? Will taking vitamins protect me from getting it?”
The question came suddenly, over coffee at an exhibition, the way these questions always seem to arrive—sideways, between small talk, from someone who has just watched a parent or a friend change into a stranger. I didn't answer with a list. I answered with a story.
The Uncle Who Was Never Like This
There was an uncle, a friend of my father’s, a brilliant engineer who had held a leading role at the Electricity Board. He retired early and settled into a quiet life with his family: television, home, rest—a different rhythm from the one he had lived for decades.
Slowly, his wife began noticing things that did not fit the man she knew. He withdrew the same amount of money from the bank twice in one day. He forgot where he had left his keys, sitting right there on his own table. He accused his wife of not making breakfast an hour after he had eaten it. Strangest of all, he began urinating on the bedroom floor, mistaking it for the toilet. And then, most painfully for a proud and dignified man, he began using crude, sexually explicit language, entirely unprovoked and entirely unlike him.
His wife and daughter were left helpless, begging for an explanation. I was a fourth-year medical student when my father told me this story. Sitting in psychiatry lectures had given me just enough to connect the dots: this was dementia.
The engineer was admitted to the psychiatric unit for a week, started on medication that did very little to slow things down, and the consultant gently prepared the family for what was coming. He went home, was cared for by his family and a caregiver, and lived only another four months.
What Dementia Actually Is
Dementia is the progressive degeneration of neurons in the brain—sometimes driven by genetics, sometimes by underlying vascular or metabolic disease, and often by both. As people live longer, it has become far more common worldwide.
It typically begins with the small things: keys, addresses, recent conversations, and financial details. Over time, it can progress to the point where a person forgets how to eat or swallow. Alongside the memory loss often comes anxiety, agitation, disrupted sleep, and behavioral change that can be as distressing to families as the memory loss itself.
The engineer’s case had all of it. And here is the uncomfortable truth people do not always want to hear: even a sharp, accomplished mind is not immune.
So What Actually Protects the Brain?
This is where the story moves from one family’s experience to the broader evidence on what may lower risk across a lifetime. In 2024, the standing Lancet Commission on Dementia Prevention, Intervention, and Care published an update that is now the most authoritative word on this question.
The Commission found compelling new evidence that untreated vision loss and high LDL cholesterol are risk factors for dementia. Altogether, it now recognizes fourteen modifiable risk factors across the life course: low education, hearing loss, hypertension, smoking, obesity, depression, physical inactivity, diabetes, excessive alcohol use, traumatic brain injury, air pollution, social isolation, vision loss, and high LDL cholesterol.
Taken together, around 45% of dementia cases worldwide are potentially preventable by addressing these fourteen factors. That number deserves to sit with you for a moment: nearly half, not through a pill, but through how we live.
Exercise and Resistance Training
Exercise, in particular, has strong and growing evidence behind it, and the story now goes beyond generic “stay active” advice. Resistance training has become one of the more promising areas of research because it may protect both memory and brain structure.
In a 2025 study from the State University of Campinas, older adults with mild cognitive impairment who completed twice-weekly weight training for six months showed improved memory, reduced atrophy in Alzheimer’s-related regions such as the hippocampus and precuneus, and better white matter integrity.
Reviews of randomized controlled trials suggest that two sessions a week for about six months may be enough to produce measurable structural benefits, possibly through increased neuroprotective growth factors such as IGF-1 and reduced inflammation.
Other Protective Habits
Beyond exercise, the same body of evidence supports what most of us already suspect:
· A well-balanced diet rich in fruits, vegetables, and fish.
· Cognitive engagement through learning, study, and unfamiliar mental challenges.
· Social connection, relaxation, travel, reading, writing, and time away from routine.
· Sleep of around seven hours a night.
· Active management of hypertension, diabetes, high cholesterol, hearing loss, and vision loss.
And What About Vitamins?
This is the part people most want to hear good news about, and it is the part where the evidence is most consistently disappointing.
A Cochrane meta-analysis pooling 28 studies and more than 83,000 participants found no quality evidence that vitamin or mineral supplements meaningfully affect cognitive decline or dementia risk, in either the short or long term.
Even large, dedicated trials confirm this. The Finnish Vitamin D Trial randomized nearly 2,500 healthy older adults to five years of vitamin D3 or placebo and found no statistically meaningful difference in dementia incidence.
The Alzheimer’s Association is direct about it: not a single food, beverage, ingredient, vitamin, or supplement has been proven to prevent, treat, or cure Alzheimer’s disease or to benefit cognitive function.
So no—there is no pill for this. Supplements may have a role when a true deficiency exists, but they cannot replace the protective effect of a life built around movement, connection, sleep, and ongoing mental challenge.
The Real Answer
What protects the brain is not found in a bottle. It is found in how you move, what you eat, how you sleep, who you talk to, and whether you keep asking your mind to do new things.
The engineer did everything right by the standards of a hardworking life: he built a career, provided for his family, and retired with dignity. What he did not do, in his quiet retirement, was keep moving, keep learning, and keep engaging.
Whether that would have changed his story, no one can say with certainty. Dementia has genetic and biological currents that lifestyle alone cannot always outrun. But the odds, the data now tells us clearly, are not neutral. They can be shifted—not by a supplement, but by a life still being lived.
